Medication Safety:
Taking your medication the right time, the right dose and the right way plays a key role in staying healthy after your liver transplant.
- Know your medications by their names, their form (capsule or liquid), size and colour;
- Be certain you can read the labels on your medication bottles and boxes;
- Try to take your medication at the same time every day;
- List and record your medication, the dosage and intake frequency in your post-transplant file;
- Use reminder tools, such as pill boxes or e-calendars with alarm, to take your medication correctly. Having your family and friends to remind you will also help;
- Be sure to follow storage directions to ensure the effectiveness of the medication;
- Keep your medications in their original containers, tightly sealed, and away from children;
- Keep track of your medication supply. Ensure that you have at least one (1) month supply of prescription before refilling it at the clinic;
- It is strongly advised that you wear a medical ID tag/ bracelet at all times with details of your medications, drug and food allergies, medical problems and emergency contact engraved on it. Otherwise, keep this list in your wallet and carry it with you at all times.
Medication Warning :
- Please take the medications as directed by your transplant team doctors and the transplant coordinator.
- Do not change, skip or stop your dose without your doctor’s approval. If you miss your doses and/ or taking it at inconsistent times, the consequences can be quite fatal and you might lose your ‘new’ liver.
- If you miss your immunosuppressant dose, it is advisable to have at least 8 hours between the current dose and the next dose.
- DO NOT DOUBLE UP ON ANY OF YOUR IMMUNOSUPPRESSANT DRUGS TO ATTEMPT TO MAKE UP FOR THE MISSED DOSE.
- Do not allow liquid medications to freeze.
- Do not take new medication, herbal or vitamin supplements/ remedies without your transplant physician’s approval.
- If you experience any unusual side effects, continuous vomitting and/ or diarrhoea, notify your transplant coordinator immediately.
